{"id":49284,"date":"2025-05-14T07:49:56","date_gmt":"2025-05-14T07:49:56","guid":{"rendered":"https:\/\/www.staging.elza-institute.com\/?p=45895"},"modified":"2025-05-31T16:45:14","modified_gmt":"2025-05-31T16:45:14","slug":"elza-egscrs-2025","status":"publish","type":"post","link":"https:\/\/www.staging.elza-institute.com\/pt\/elza-egscrs-2025\/","title":{"rendered":"Inova\u00e7\u00f5es no tratamento biomec\u00e2nico do ceratocone avan\u00e7ado na EgSCRS"},"content":{"rendered":"<p>Os recentes avan\u00e7os no cross-linking corneano (CXL) e no diagn\u00f3stico biomec\u00e2nico expandiram o horizonte de tratamento do ceratocone avan\u00e7ado e da ectasia p\u00f3s-LASIK.<\/p>\n<p>Na recente reuni\u00e3o da Sociedade Eg\u00edpcia de Catarata e Cirurgia Refractiva (EgSCRS) <a href=\"https:\/\/egscrs-conferences.org\/\">confer\u00eancia<\/a>, Diretor Executivo da ELZA,\u00a0<a href=\"https:\/\/www.staging.elza-institute.com\/pt\/sobre\/nikki-hafezi\/\">Nikki Kristoffersen-Hafezi<\/a>, MAS IP ETHZ e CMO da ELZA,\u00a0<a href=\"https:\/\/www.staging.elza-institute.com\/pt\/sobre\/farhad-hafezi\/\">Prof. Farhad Hafezi<\/a>O Dr. G., MD, PHD, FARVO, explicou por que raz\u00e3o \u00e9 t\u00e3o importante o rastreio e o tratamento port\u00e1teis e econ\u00f3micos de doen\u00e7as da c\u00f3rnea que podem provocar a cegueira. Eles tamb\u00e9m explicaram por que desenvolveram novos protocolos, como ELZA-sub400, ELZA-PACE e ECO-CAIRS, para oferecer novas op\u00e7\u00f5es terap\u00eauticas para o tratamento de c\u00f3rneas de ceratocone ultrafinas, cones altamente irregulares e estabiliza\u00e7\u00e3o biomec\u00e2nica sem remo\u00e7\u00e3o de tecido. Esta publica\u00e7\u00e3o descreve uma abordagem passo a passo, biomecanicamente informada, para a reabilita\u00e7\u00e3o do ceratocone, integrando diagn\u00f3sticos, triagem assistida por IA e uma pir\u00e2mide de interven\u00e7\u00f5es em etapas.<\/p>\n<h2>Combater a cegueira evit\u00e1vel com tecnologia escal\u00e1vel<\/h2>\n<p>O peso global do ceratocone n\u00e3o diagnosticado - especialmente em crian\u00e7as e adolescentes - \u00e9 impressionante. Testemunhei em primeira m\u00e3o como o acesso continua a ser uma barreira importante. Para resolver este problema, a Light for Sight Foundation desenvolveu uma plataforma de diagn\u00f3stico integrada na nuvem e apoiada por IA, constru\u00edda em torno de uma vers\u00e3o miniaturizada do CSO MS-39 e de uma arquitetura de carregamento r\u00e1pido. Este sistema, mostrado aos delegados da EgSCRS e representado na Figura 1, permite o rastreio em massa em locais com poucos recursos e integra-se com plataformas de aprendizagem autom\u00e1tica.<\/p>\n<p><a href=\"https:\/\/www.staging.elza-institute.com\/pt\/handheld-sbk\/\" rel=\"attachment wp-att-45844\"><img fetchpriority=\"high\" decoding=\"async\" class=\"aligncenter size-medium wp-image-45844\" src=\"https:\/\/www.staging.elza-institute.com\/wp-content\/uploads\/2025\/05\/Handheld-SBK-300x200.jpg\" alt=\"Querat\u00f3grafo baseado em smartphone (SBK)\" width=\"300\" height=\"200\" \/><\/a><\/p>\n<p><strong>Figura 1:<\/strong> O prot\u00f3tipo de 2020 da unidade de rastreio port\u00e1til.<\/p>\n<p>A import\u00e2ncia do acesso ao tratamento \u00e9 fundamental: o diagn\u00f3stico sem a possibilidade de interven\u00e7\u00e3o pode fazer mais mal do que bem. Temos enfatizado este ponto repetidamente - se uma crian\u00e7a \u00e9 diagnosticada com ceratocone, o tratamento deve seguir-se, independentemente da capacidade financeira. O rastreio s\u00f3 tem valor quando conduz \u00e0 a\u00e7\u00e3o.<\/p>\n<h2>Repensar o diagn\u00f3stico biomec\u00e2nico<\/h2>\n<p>Os delegados do EgSCRS ouviram que a tomografia Scheimpflug padr\u00e3o continua a ser fundamental, mas as suas limita\u00e7\u00f5es no ceratocone precoce ou subcl\u00ednico est\u00e3o bem estabelecidas. Ferramentas como o Corvis ST oferecem informa\u00e7\u00f5es biomec\u00e2nicas valiosas, mas tecnologias emergentes como a microscopia de Brillouin e a elastografia OCT est\u00e3o a inaugurar uma nova era.<\/p>\n<p>Uma das nossas principais direc\u00e7\u00f5es de investiga\u00e7\u00e3o tem sido o desenvolvimento de verdadeiros mapas de calor biomec\u00e2nicos baseados na deforma\u00e7\u00e3o localizada e na subsequente resposta da c\u00f3rnea. Estes mapas, que podem revelar diferen\u00e7as regionais de rigidez em c\u00f3rneas queratoc\u00f3nicas, est\u00e3o agora a entrar nos primeiros ensaios em humanos.<\/p>\n<p><a href=\"https:\/\/www.staging.elza-institute.com\/pt\/oct-elastography-image\/\" rel=\"attachment wp-att-45845\"><img decoding=\"async\" class=\"aligncenter size-medium wp-image-45845\" src=\"https:\/\/www.staging.elza-institute.com\/wp-content\/uploads\/2025\/05\/OCT-elastography-image-300x233.jpg\" alt=\"OCT-elastografia-imagem\" width=\"300\" height=\"233\" \/><\/a><\/p>\n<p><strong>Figura 2:<\/strong> Rigidez diferencial da imagem de elastografia OCT entre o cone e a periferia numa c\u00f3rnea queratoc\u00f3nica.<\/p>\n<h2>ELZA-sub400: Adapta\u00e7\u00e3o da flu\u00eancia \u00e0 espessura residual do estroma<\/h2>\n<p>As diretrizes tradicionais defendiam que o CXL em c\u00f3rneas mais finas do que 400 \u00b5m n\u00e3o era seguro. No entanto, como foi referido pelos participantes na EgSCRS, este limiar \u00e9 uma simplifica\u00e7\u00e3o excessiva. A vari\u00e1vel-chave \u00e9 a profundidade da flu\u00eancia e n\u00e3o a paquimetria absoluta. Em 2016, introduzimos um algoritmo de modula\u00e7\u00e3o da flu\u00eancia que ajusta a energia UV-A total com base na espessura da c\u00f3rnea e na cin\u00e9tica de difus\u00e3o do oxig\u00e9nio.<\/p>\n<p>Este modelo, agora integrado no EMAGine <a href=\"https:\/\/www.emagine-eye.com\/c-eye-device\/\"><strong>Dispositivo C-eye<\/strong><\/a>permite uma reticula\u00e7\u00e3o segura em c\u00f3rneas com uma espessura de 200 \u00b5m. O protocolo, denominado <a href=\"https:\/\/www.staging.elza-institute.com\/pt\/corneal-cross-linking-overview\/cross-linking-cxl-thin-cornea\/\"><strong>ELZA-sub400<\/strong><\/a>O sistema 82%, que \u00e9 um dos mais recentes produtos de corre\u00e7\u00e3o de c\u00f3rneas, atingiu agora a sua segunda gera\u00e7\u00e3o. Os resultados cl\u00ednicos mostram uma taxa de estabiliza\u00e7\u00e3o do 82% aos 2 anos, mesmo em c\u00f3rneas com Kmax &gt;90 D e espessura do estroma &lt;300 \u00b5m. \u00c9 importante salientar que a densidade das c\u00e9lulas endoteliais n\u00e3o \u00e9 afetada quando se utiliza uma flu\u00eancia devidamente titulada.<\/p>\n<p><strong><br \/>\n<a href=\"https:\/\/www.staging.elza-institute.com\/pt\/cxl-in-thin-corneas-with-the-sub400-protocol-paper-published\/individualized-method-sub400\/\" rel=\"attachment wp-att-34557\"><img decoding=\"async\" class=\"aligncenter wp-image-34557 size-large\" src=\"https:\/\/www.staging.elza-institute.com\/wp-content\/uploads\/2021\/03\/Individualized-method-sub400-1024x361.jpg\" alt=\"\" width=\"800\" height=\"282\" srcset=\"https:\/\/www.staging.elza-institute.com\/wp-content\/uploads\/2021\/03\/Individualized-method-sub400-1024x361.jpg 1024w, https:\/\/www.staging.elza-institute.com\/wp-content\/uploads\/2021\/03\/Individualized-method-sub400-300x106.jpg 300w, https:\/\/www.staging.elza-institute.com\/wp-content\/uploads\/2021\/03\/Individualized-method-sub400-768x271.jpg 768w, https:\/\/www.staging.elza-institute.com\/wp-content\/uploads\/2021\/03\/Individualized-method-sub400-1536x541.jpg 1536w, https:\/\/www.staging.elza-institute.com\/wp-content\/uploads\/2021\/03\/Individualized-method-sub400-2048x722.jpg 2048w, https:\/\/www.staging.elza-institute.com\/wp-content\/uploads\/2021\/03\/Individualized-method-sub400-16x6.jpg 16w\" sizes=\"(max-width: 800px) 100vw, 800px\" \/><\/a><br \/>\nFigura 3:<\/strong> O algoritmo ELZA-sub400 modula a flu\u00eancia UV-A com base na espessura residual do estroma, reduzindo o fornecimento de energia em c\u00f3rneas ultrafinas.<\/p>\n<h2>PACE: Modula\u00e7\u00e3o topogr\u00e1fica de precis\u00e3o sem abla\u00e7\u00e3o do estroma<\/h2>\n<p>Embora os protocolos Athens e CREST tenham demonstrado excelentes resultados na remodela\u00e7\u00e3o da c\u00f3rnea atrav\u00e9s da combina\u00e7\u00e3o de abla\u00e7\u00e3o com excimer e CXL, envolvem a remo\u00e7\u00e3o do estroma - uma preocupa\u00e7\u00e3o no ceratocone avan\u00e7ado.<\/p>\n<p><a href=\"https:\/\/www.staging.elza-institute.com\/pt\/ceratocone\/pace-customized-cross-linking-cxl\/\"><strong>ELZA-PACE<\/strong><\/a> (reticula\u00e7\u00e3o epi-on personalizada assistida por PTK) \u00e9 uma abordagem fundamentalmente diferente. Utilizando o mapa de espessura epitelial do MS-39 e a an\u00e1lise do perfil de altura, definimos uma zona de remo\u00e7\u00e3o epitelial topograficamente personalizada sobre o \u00e1pice do cone utilizando PTK. Nenhum tecido do estroma \u00e9 ablacionado.<\/p>\n<p>Os delegados do EgSCRS foram informados de que isto cria uma janela de epi-off altamente controlada, estabelecendo gradientes de oxig\u00e9nio, concentra\u00e7\u00e3o de riboflavina e flu\u00eancia - que amplificam o efeito de achatamento a n\u00edvel central. O PACE foi utilizado em mais de 300 olhos com resultados que incluem at\u00e9 seis linhas de Snellen ganhas com \u00f3culos. O procedimento requer:<\/p>\n<ul>\n<li>MS-39 para mapeamento epitelial<\/li>\n<li>Plataforma de excimer AMARIS da Schwind<\/li>\n<li>Dispositivo C-eye<\/li>\n<li>RIBO-Ker riboflavina com potenciadores de penetra\u00e7\u00e3o<\/li>\n<\/ul>\n<h2>ECO-CAIRS: Repensar a ICRS com segmentos alog\u00e9nicos reticulados<\/h2>\n<p>Os segmentos de anel corneano alog\u00e9nicos (CAIRS), tal como introduzidos por Susan Jacob, oferecem uma alternativa biol\u00f3gica aos ICRS de PMMA. No entanto, a variabilidade dos dadores e os desafios de manuseamento intra-operat\u00f3rio t\u00eam limitado a sua ado\u00e7\u00e3o. Introduzimos <a href=\"https:\/\/www.staging.elza-institute.com\/pt\/tag\/eco-cairs\/\"><strong>ECO-CAIRS<\/strong><\/a>, aplicando reticula\u00e7\u00e3o extracorporal aos segmentos dadores com flu\u00eancia ultra elevada (at\u00e9 62 J\/cm\u00b2) antes da implanta\u00e7\u00e3o.<\/p>\n<p>Isto oferece m\u00faltiplas vantagens:<\/p>\n<ul>\n<li>Maior rigidez do segmento para efic\u00e1cia biomec\u00e2nica<\/li>\n<li>Redu\u00e7\u00e3o do incha\u00e7o e maior facilidade de implanta\u00e7\u00e3o<\/li>\n<li>Elimina\u00e7\u00e3o de querat\u00f3citos para seguran\u00e7a imunol\u00f3gica<\/li>\n<\/ul>\n<p>Os dados experimentais confirmam o aumento da rigidez e a resist\u00eancia ao incha\u00e7o. As inser\u00e7\u00f5es s\u00e3o tecnicamente mais f\u00e1ceis e os segmentos permanecem est\u00e1veis no p\u00f3s-operat\u00f3rio. O ECO-CAIRS \u00e9 agora a base da pir\u00e2mide cir\u00fargica ELZA:<\/p>\n<ol>\n<li><strong>ECO-CAIRS<\/strong>: para suporte mec\u00e2nico em cones avan\u00e7ados<\/li>\n<li><strong>ELZA-PACE<\/strong>: para uma normaliza\u00e7\u00e3o topogr\u00e1fica orientada<\/li>\n<li><strong>TransPRK guiada por frentes de onda<\/strong>: para refinamento \u00f3tico<\/li>\n<\/ol>\n<p>Esta abordagem faseada permite a reabilita\u00e7\u00e3o da vis\u00e3o mesmo em olhos anteriormente considerados adequados apenas para a ceratoplastia.<\/p>\n<p><a href=\"https:\/\/www.staging.elza-institute.com\/pt\/pyramid-pace\/\" rel=\"attachment wp-att-42441\"><img loading=\"lazy\" decoding=\"async\" class=\"aligncenter size-full wp-image-42441\" src=\"https:\/\/www.staging.elza-institute.com\/wp-content\/uploads\/2024\/03\/pyramid-pace.svg\" alt=\"\" width=\"486\" height=\"491\" \/><\/a> <a href=\"https:\/\/www.staging.elza-institute.com\/pt\/pyramid-elza-pace\/\" rel=\"attachment wp-att-45846\"><img loading=\"lazy\" decoding=\"async\" class=\"aligncenter size-medium wp-image-45846\" src=\"https:\/\/www.staging.elza-institute.com\/wp-content\/uploads\/2025\/05\/pyramid-elza-pace-1.svg\" alt=\"\" width=\"1\" height=\"1\" \/><\/a><\/p>\n<p><strong>Figura 5:<\/strong> A pir\u00e2mide cir\u00fargica da ELZA<\/p>\n<h2>Conclus\u00e3o<\/h2>\n<p>Os participantes do EgSCRS foram informados sobre como o tratamento do ceratocone est\u00e1 entrando em uma nova era - uma era definida pela personaliza\u00e7\u00e3o, modula\u00e7\u00e3o biomec\u00e2nica em etapas e invasividade m\u00ednima. Com diagn\u00f3sticos apoiados por IA, CXL adapt\u00e1vel \u00e0 flu\u00eancia (ELZA-sub400), remodela\u00e7\u00e3o topogr\u00e1fica n\u00e3o ablativa (ELZA-PACE) e ICRS aprimorado ex vivo (ECO-CAIRS), agora podemos oferecer interven\u00e7\u00f5es personalizadas para quase todos os est\u00e1gios da doen\u00e7a.<\/p>\n<p>Estas inova\u00e7\u00f5es n\u00e3o s\u00e3o isoladas - representam uma estrat\u00e9gia integrada.
